Most arrival cards and declarations are needed by every traveller, including children, so the count multiplies with the size of your group. A family of four visiting two countries in Southeast Asia can easily need 12 or more forms before they come home.

Do children need their own arrival card?
For most countries, yes. Thailand's TDAC, Malaysia's MDAC and the All Indonesia card each need one per traveller, including children, though some let one person submit for the group.
Does this count the forms for coming home?

Are government fees included?
Yes. Paid forms, like ETAs, e-visas and levies, are shown with their official cost. You pay those to each government directly.
